SPRAVATO® Treatment: Insurance, Costs & How to Get Started
If you are considering SPRAVATO® (esketamine) nasal spray for treatment-resistant depression, it is natural to have questions about the process, timeline, and out-of-pocket costs.
The path to treatment involves a coordinated step-by-step workflow. We have structured this process to resolve your insurance coverage and out-of-pocket costs before your clinical treatment plan is finalized.

Step 1: Verify Your Insurance Benefits (Two-Step Verification)
To get a complete, down-to-the-penny picture of your coverage, you must complete both steps below. One verifies the cost of the medication itself, and the other verifies the cost of your 2-hour clinical stay.
If you have commercial health insurance, call the manufacturer's care navigators directly at 1-844-4S-WITHME (1-844-479-4846).
-
What they do: They will run your commercial card over the phone instantly to verify if your policy covers the nasal spray itself and confirm your eligibility for the $10 medication copay card.
Verify Your Medication Cost (Janssen Care Navigators)
Part 1:
The medication line cannot check your medical observation benefits. You must call the Member Services number on the back of your insurance card and use this exact script to check your facility and clinical monitoring coverage:
"Hello, I am calling to check my outpatient medical benefits for SPRAVATO® clinical monitoring. Could you please verify the following for me?
-
What is my out-of-pocket responsibility (copay or coinsurance) for a mandatory 2-hour in-office clinical observation window?
-
Does your plan prefer individual outpatient codes (CPT codes 99215 and 99415) or the bundled healthcare codes (HCPCS codes G2082 and G2083) for this observation time?"
Verify Your Clinical Observation Cost (Your Insurance Provider)
Part 2:
Step 2: Schedule Your Initial Consultation

Initial Clinical Evaluation: You will meet with Dr. Thurston to review your medical history, confirm your diagnostic eligibility, and determine if SPRAVATO® is the right clinical path for you.
Once you have reviewed your baseline benefits and have a general idea of your copay responsibilities, you are ready to begin the formal clinical onboarding process:
If you are a candidate and choose to move forward, you must review and initial our formal Treatment Agreement & Commitment Contract before leaving the clinic. Because securing specialized insurance clearance requires a significant investment of clinical and administrative hours, our billing team will not begin the prior authorization process until this signed commitment is on file. This contract outlines your commitment to the strict twice-weekly induction schedule, the mandatory 2-hour observation window, and our strict 48-hour cancellation policy.
The Treatment Agreement & Commitment Contract:

Step 3: Post-Evaluation Enrollment & Next Steps
Only after your initial evaluation is complete and your internal commitment contract is signed will we move forward with your insurance submission.
-
Complete Your Support Enrollment: Our front desk will hand you an office tablet (or email you a secure link) to complete your digital registration on the SPRAVATO withMe Patient Support Portal. Signing this digital authorization form sends a secure alert directly to our provider dashboard, giving our billing team the green light to act.

Prior Authorization (PA) Submission: Our billing team will immediately submit your clinical notes, signed disclosure forms, and documentation to your insurance company to secure the required Prior Authorization.
-
Mandatory REMS Enrollment: Once your insurance company officially greenlights the PA, our team will enroll you in the mandatory SPRAVATO REMS (Risk Evaluation and Mitigation Strategy) safety database. This is the final federal requirement that allows us to allocate your medication inventory for your first session.

Understanding Your Real Treatment Costs
The numbers cited online for SPRAVATO® can be intimidating, but health insurance and manufacturer support programs dramatically reduce the actual cost for most patients.
Pathway A: For Patients with Commercial Insurance
If you have commercial health insurance, you are eligible to enroll in two distinct cost-saving programs offered by SPRAVATO withMe to offset both the drug and the clinical visit costs.
1. The SPRAVATO withMe Medication Savings Program
This program targets the out-of-pocket cost of the nasal spray itself.
-
Your Cost: Eligible patients pay as low as $10 per treatment for the SPRAVATO® medication.
-
How It Works: Once our office secures your insurance Prior Authorization (PA), we will activate your savings card. Our billing team processes this card as secondary coverage alongside your primary insurance.
-
Program Caps: Covers up to 25 treatments per calendar year, with a maximum annual drug program benefit of $7,150.
-

You can also enroll by calling: 844-4S-WITHME (844-479-4846), Monday–Friday, 8:00 AM–8:00 PM ET.

Note: the withMe program is for commercial insurance only. If you have Medicare, government insurance of any kind or no insurance you can visit:
-
-
2. The SPRAVATO withMe Observation Copay Savings Program
Because the medication card only covers the chemical spray, Janssen provides a separate program to help cover your out-of-pocket costs for the mandatory 2-hour clinical observation window (the clinical staff and facility time).
-
Your Cost: You can receive up to $150 per treatment session to cover your primary insurance copay or coinsurance for the observation codes.
-
How It Works: After your treatment, our clinic bills your commercial medical insurance for the observation time. Once your primary insurance pays their portion and assigns your remaining copay, the Observation Support program kicks in to cover up to $150 of that balance.
-
Program Caps: Covers up to a maximum of $3,750 in clinical observation cost-sharing per calendar year.
Pathway B: For Patients with Medicare / Government-Funded Coverage
Federal regulations prohibit patients with government-funded insurance (including traditional Medicare, Medicare Advantage, Medicaid, or TRICARE) from utilizing manufacturer copay cards for either the medication or observation. However, coverage pathways remain highly accessible:
-
Medicare Part B Coverage: SPRAVATO® is typically covered under Medicare Part B (Medical Insurance) rather than Part D (Pharmacy). Medicare Part B generally covers 80% of the approved amount for both the medication and the observation time once your annual deductible is met.
-
Supplemental / Medigap Policies: If you have a supplemental insurance policy, it will often cover the remaining 20% coinsurance balance, leaving you with little to no out-of-pocket costs per session.
-
Alternative Financial Support: If you do not have supplemental coverage and face high out-of-pocket costs, you can explore independent, non-profit charitable assistance foundations. To review these options, visit the SPRAVATO withMe Medicare Support Page to check current fund openings and eligibility requirements.
